I-9 Employment Eligibility Verification
WHO SHOULD FILE THIS FORM: All employees, citizens and
noncitizens, hired after November 6, 1986, must complete Section
1 of this form at the time of hire, which is the actual
beginning of employment. Employers must complete Section 2 by
examining evidence of identity and employment eligibility within
three (3) business days of the date employment begins.

IT-2104 Employee's Withholding Allowance Certificate
WHO SHOULD FILE THIS FORM: This certificate, Form IT-2104, is
completed by an employee and given to the employer to
instruct the employer how much New York State (and New York
City and Yonkers) tax to withhold from the employee’s pay.
The more allowances claimed, the lower the amount of tax
withheld.

WHO QUALIFIES: You may claim exemption from
withholding if: (1) you are under age 18, over age 65, or a
full-time student under age 25, and (2) you had no New York
income tax liability for 2008, and (3) expect none for the

your return shows tax
before the allowance of any credit for income tax withheld.)

THIS FORM: Form 5305-EA is a model custodial account
agreement that meets the requirements of section 530(b)(1) and
