Health Industry Resources Enterprises, Inc. (HIRE, Inc.), a non-profit community based organization, leader in providing mental health, housing and social services for individuals suffering from HIV/AIDS, is seeking CASE MANAGERS for its Scatter-Site Program.

• Provide support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services.
• Maintain regular personal contact with clients in order to monitor physical and emotional capacity for independent living.
• Visit the client in their homes on a weekly basis.
• Formulate needs assessment and implement service plan.
• Document all contracts made with the client and an contact on their behalf
• Meet with the Program Supervisor every week.
• Participate in training, workshops, seminars and meetings as mandated.

• Bachelor's Degree with one (1) year of work experience in the field of health and human services or an Associate's Degree plus three (3) years experience in providing counseling to this population.

SALARY:              Commensurate with experience.
