To enroll your child in any of our programs you must pick up an application from the Hostos Children’s Center. We are located on the first level of the A-Building, room 109A. Or, you may download the application here, complete, and email it to childrenscenter@hostos.cuny.edu.
The following documents listed below are also required.
- Current Medical Exam (must be completed by your doctor; document must be less than a year old)
- Current Immunization Record
- Day Care Cumulative Health Record (Yellow Form)
- Child’s Birth Certificate
- Current Government Issued Photo ID (For example; driver’s License, State ID, Passport/Visa, ID NYC, Green Card, or Permanent Residency Card)
- Income Verification (For example; current Income Tax, Pay Stub, Child Support Letter, etc.)
- Once the documents are submitted a staff member will review and an appointment for scheduling and tuition agreement will be made.
